Facial animations.

So not sure if this IS a bug, thing, or misshap but I want to put it out there that this happens (whenever he enters combat).

What went wrong with facial animations when going from CC to actual game?

It is a bug. If you fiddle with the expression slider and then went on to change something like body type, the slider would change it self. You have to make sure that expressions is the last thing you change before going into game otherwise it will glitch.
I found the bug in CBT2 and was really annoyed by it too but atm, this seems like the safest walk around...

I never changed the idle face animation, this is whenever he enters combat. Just to make sure I checked it twice by making duplicate characters with and without the facial animation in game being affected and it still ends up doing that face.

The face expressions appear to be dynamic in this game. The change depending on action like fishing, standby and combat. I guess that is what we see in your screenshot but it seems its a bit to the extreme for you.
The changes in my own character are not as far drastic. She looks more like a confused fat child for example when fishing. Its hard to say whenever your case is a bug or actual mechanic
